Facility Requests
Please use the handy form below to submit a request to the office for use of the church facility or van. Every organization or activity leader must submit a facility use request before scheduling any activity in order to avoid conflicts. A paper form may also be found in the Communication Center at the church. Additional information may be requested after your needs have been reviewed. You must receive confirmation of availability of the facility before your acitivity can be officially scheduled on the church calendar.
If you wish for your activity to be inlcuded in the bulletin or website announcements, please also complete an announcement form.

'Authorized Driver' must have passed the Guide One Driver Course & have a current drivers license on file with the Church Office.
